This Agreement is made today between the Coach of the Program (Theresa Sailor) and You [the Client].

PAYMENTS AND REFUNDS
The Client understands that in person services are due upon receipt and may be billed at time of scheduling with internet booking system.
Sessions via video conference will be billed at time of scheduling.
Cancellation of services within 24 hours of a scheduled appointments will not be refunded except at the sole discretion of Theresa Sailor.
Cancellation of services within 24 hours of a scheduled appointments by your coach, Theresa Sailor will result in you receiving a full refund of the purchase price OR the option to reschedule your appointment with no additional charge.
If our video conference is interrupted due to technical issues not caused by failure to follow the instructions listed below, we can either:
Switch to a phone consultation
Cancel the consultation and be refunded for the full amount
Given the nature of technology, specifically video conferencing, there is always the possibility of a poor connection or other issues that could make this form of consultation ineffective.
You agree to prepare for our consultation by doing the following things:
Use an approved browser: Chrome, Firefox, Opera or Vivaldi
Be sure your browser is properly updated
Close all open windows, files and applications before entering the video conference room
Turn off any other devices that may be using your wifi signal or bandwidth
If you are a heavy internet user, especially if you watch a lot of video: please restart your device before the consultation begins
Disable incoming calls, notifications and messaging before we begin
If, after following the instructions to prepare; our consultation is interrupted or scrambled due to other technical or unforeseen issues, we can reschedule, or switch to a phone consultation and continue, or cancel.
In the event our session is cancelled due to technical issues and you prepared by following the above instructions, you will receive a refund based on percentage of the session completed.
It is my policy to begin and end sessions at the scheduled time. If you are unable to attend, please contact me within 24 hours of appointment to avoid being charged the full amount for the appointment. If you are late for your appointment, extra time cannot be given to make up for it.
It is very important that you bring with you (if your session is in person) or return (If your session is via internet conference) any forms or assignments in order for our sessions to be used for their intended purpose.
If this is not done, it may be necessary to use your scheduled session time to complete it/them, extra time will not be given for this purpose.
The Client understands that the role of the Health Coach is not to prescribe or assess micro- and macronutrient levels; provide health care, medical or nutrition therapy services; or to diagnose, treat or cure any disease, condition or other physical or mental ailment of the human body.
Rather, the Coach is a mentor and guide who has been trained in integrative, holistic wellness coaching to help clients reach their own health goals by helping them to devise and implement positive, sustainable lifestyle changes.
The Client understands that the Coach is not acting in the capacity of a doctor, licensed dietitian, nutritionist, psychologist or other licensed or registered professional, and that any advice given by the Coach is not meant to take the place of advice by these professionals.
If the Client is under the care of a licensed medical professional or currently uses prescription medications, the Client should discuss any dietary changes or potential dietary supplement use with his or her licensed medical professional, and should not discontinue any prescription medications without first consulting his or her licensed medical professional.
The Client has chosen to work with the Coach and understands that the information received should not be seen as medical or nursing advice and is not meant to take the place of seeing licensed medical professionals.
PERSONAL RESPONSIBILITY AND RELEASE OF HEALTH CARE RELATED CLAIMS
The Client acknowledges that the Client takes full responsibility for the Client’s life and well-being, as well as the lives and well-being of the Client’s family and children (where applicable), and all decisions made during and after this consultation.
The Client expressly assumes all the risks, real or perceived of the Consultation, including the risks of trying new foods or supplements, and the risks inherent in making lifestyle changes.
The Client releases the Coach from any and all liability, damages, causes of action, allegations, suits, sums of money, claims and demands whatsoever, in law or equity, which the Client ever had, now has or will have in the future against the Coach, arising from the Client’s past or future participation in, or otherwise with respect to, the Consultation, unless arising from the gross negligence of the Coach.
The Coach will keep the Client’s information private, and will not share the Client’s information to any third party unless compelled
to by law.
You agree to receive and accept email or text reminders of our scheduled appointments and to receive, complete and return questionnaires and forms relating to our appointments.
